STEPS

1. Prepare necessary documents (shown below)
2. Contact to WAFCAI and send documents via E-mail or Post mail also welcome to visit directly WAFCAI office.
3. Check documents and examine conditions by social worker.
4. Set up a schedule for assessment and fitting.
5. Meet at WAFCAI office or an appointed site.
6. Provide wheelchairs (following 8-step provision).

NECESSARY DOCUMENTS

1. Copy of family card
2. Copy of ID card (parents or client)
3. Income statement/salary slip (Non-employee: Income certificate signed by local RT)
4. Electric Account (if have)
5. Full body photo of the client (free size)
6. Domicile letter (the contract or boarding house only)
